CULTURAL CONSIDERATIONS:
American - Family structure: Nuclear, extended and single parent households. Often family friends
are referred to with familiar pronouns, i.e. Uncle, Aunt, etc.
Those with African-American cultural background may ascribe to the theory of humoral health, which
focuses on balance or harmony with nature. The cultural belief is that if there is imbalance with
nature, the result can be illness or disease; the treatment of disease with hot or cold substances
restores the body’s humoral balance. To restore this balance for their children, parents may ask the
nurse to support their cultural beliefs by providing hot or cold substances, including hot or cold
drinks, depending on the child’s illness. African Americans are usually cautious around and
distrustful of the majority population, value direct eye contact, and use nonverbal expressions.
Need for Calcium in African-American: Foods high in calcium are important for children for the
growth of bones. When a child sustains a fracture, the need for calcium may be increased. A major
source of calcium is dairy products. Many African Americans have a lactose intolerance in which
they are unable to digest lactose, and ingestion of dairy foods causes abdominal bloating, diarrhea,
and excessive flatus. Therefore, when they experience a fracture, they may need to obtain calcium
from other food sources such as green leafy vegetables, broccoli, cauliflower, and cabbage. If they
do consume dairy products, they may need to take a commercially available lactose preparation
such as Lact- Aid or Dairy-Ease. (Potts p. 611, 1304)
