RESEARCH PROJECT PROPOSAL Research Project Proposal The fall rate for elderly patients who receive services in the home hospice environment has risen and overtime it is developing into more of a concern. An essential position that the healthcare provider participates in is promoting and encouraging safety within the home hospice environment. The roles of the Nurse Practitioner (NP) consist of performing face to face and teaming up or working jointly with the interdisciplinary team to provide appropriate care to patients (Greene, 2016). The responsibility of the NP also consist of recognizing and finding appropriate tactics to implement so that safe care can be delivered to the patient in the home environment to prevent or decrease further falls from occurring. Being knowledgeable about factors that lead to patient safety occurrences or incidents is essential for the healthcare professional, as well as, the patient and family member so that appropriate interventions are implemented to help eradicate the issue. The greatest quality of care and a decrease in patient safety problems can be promoted when the appropriate interventions are placed in the beginning prior to a patient fall. Background and Significance of the Problem Falls in the elderly are usually frequent and critical for older residents who have fallen and receive services in the home environment. The falls frequently result in injuries and death of the elderly. Falls remain to be an uncertain patient safety issue in the general communities in addition to the outpatient and inpatient setting. Falls and associated injuries are progressively gaining more recognition as a sensitive quality nursing indicator (Dionyssiotis, 2012). A significant quantity of all sentinel proceedings is related to patient falls. It was noted by the Joint Commission analysis of sentinel proceedings between 1995- 2012, falls are the sixth leading reason of unexpected events and make up 7.6% of complete sentinel events (Dionyssiotis, 2012).2 RESEARCH PROJECT PROPOSAL More than one third of older adults older than 65 years of age will fall every year. A lot of falls are significantly avoidable that occur from the elderly (Health Stream, 2015). The importance of this study will determine whether or not putting into place a multi factorial or complex fall risk assessment on admittance will produce an affirmative result on decreasing falls within the home hospice environment. Statement of the Problem and Purpose of the Study The problem statement is the escalated fall rate taking place in the elderly home hospice environment; healthcare providers are neglecting to begin a fall risk assessment on admission to seek risk factors that enhance a patient’s risk of falling. Identifying whether or not implementing a fall risk review on admission will prevent or decrease the elderly from falling in the home hospice environment is the purpose of this investigation. Literature Review Older adults who receive care in the home environment often ensue falls the cause’s serious injuries and in some cases death (Robertson et al., 2012). Falls in the elderly are usually frequent and critical for older residents who have fallen and receive services in the home environment. Two out of the four studies reviewed gathered information using mutually a quantitative as well as qualitative research. Patricia Benner’s Theory is the theory used to compare the researches. Each study utilized observational research to distinguish diverse ways to proceed so that risk influences or approaches can be focused in on to implement reduction or avoid home environment patient safety occurrences. Harriosn et al. (2013) recognized and developed an approach to attain information. In the home environment there was adverse occurrences recognized throughout the examination.
